My experience is that you don't. Email from IMDA:
"Since you already have a valid radio amateur station licence with IMDA, you as an individual can import the radio amateur equipment provided the equipment can be accepted for use locally."
This arose when I gave up trying to purchase an FT-817ND from a local dealer (the factory kept stringing him along for months) and wanted to import one myself.
Obviously IMDA needs to know what equipment comprises your station. I sought consent prior to import, then notified serial# (provided an updated equipment list with the serial# added) once I received the unit.
